Your Guide
Intoduction
"Your Guide" section is an easy and complete reference of important information you need to know about your Health Care Agreement. This section has been prepared specially to guide you on how to avail of the preventive, diagnostic and treatment services enumerated in your contract.

Upon approval of your application for membership with PHILAMCARE, you will be provided with a PhilamCare Health Care Kit comprised of the following:

1. Your Health Care Agreement
2. Your PhilamCare Membership Card
3. This "Your Guide" procedure booklet
4. Updated list of PhilamCare out-patient clinics and affiliated hospitals

The Health Care Agreement, together with the application form, AD & D Insurance certificate (if applicable), Full Service Hospital and other endorsements, constitute your health care services contract with PhilamCare. In order for you to maximize your availment of benefit entitlements, you are advised to read and understand the Agreement thoroughly.

If you are a member of a Group Coverage, your Health Care Agreement is contained in a Master Contract issued to your employer. This is a voluminous document which cannot be included here.

PhilamCare is committed to serve you. You can help us meet this commitment by familiarizing yourself with your health care plan and letting us know of any difficulty you may encounter when availing of your benefit.

Hospital Coordinators and Liaison Officers assigned to the different PhilamCare affiliated hospitals will look into your health plan related needs during your confinement.

Finally, our Membership Relations Department shall also be glad to help you with any problem that may arise concerning your health care plan.
